Use the space below to tell us more about yourself and your availability. You will receive a confirmation email after completing the form below. Once we receive your information, we will create an account on  our teletherapy system, Simple Practice and then send you an important link for forms and waivers. At the same time, we will send you an appointment time based on your listed availability. If you have additional questions or concerns, please email counseling@mydearscout.com.

Are you under 18? We provide sevice for over AND under 18, this just helps us get you the right paperwork.
